DISTINGUISHED SERVICE AWARD NOMINATION

It is time again to submit your nomination for the Distinguished Service Award.

This award is given to a past or present Full Member of the Association who has or is making significant contributions to the growth, development, and professional stature of the Purchasing Management Association of Western New England, and the Purchasing profession.

Past awardees have included Jeffrey Luty, C.P.M., John Coolidge, C.P.M.,
Sue Barlow, and Miguel Hollander.

Some of the factors that are to be considered in the judging of each candidate's application are:

1. Participation in scheduled meetings, seminars and other
activities of the Association.
2. Service on Association committees or groups.
3. Service on the Association's Board of Directors, as
either an Officer or a Director.
4. Contributions to the Institute for Supply Management at
the Northeast Supply Management Group and I.S.M. level.
5. Adherence to the I.S.M.'s "Principles and Standards of
Purchasing Practice."

Please complete the cover page attached, and submit a detailed sketch including services rendered on an association, group, or national level, committees chaired or served on, offices held, and all other pertinent data that would make your nominee an outstanding winner. Nominations should be forwarded to the above address, with "D.S.A." printed/written on the front of the envelope by March 1, 2009.
